Shared Care Agreement Pathway: ADHD, Autism & General Psychiatric Care

A Shared Care Agreement is a structured partnership between Minds Matter Clinic and a patient’s GP to support the safe and coordinated management of ongoing mental health treatment—including ADHD, Autism (where pharmacological management is appropriate), and general psychiatric conditions (e.g., anxiety, depression, mood disorders).

Eligibility Criteria for Shared Care

A patient becomes eligible for shared care when:

A full diagnostic assessment has been completed.
Medication has been initiated and managed by a prescribing clinician.
The patient has attended a minimum of three review appointments.
Confirmed stable and effective medication response, No serious adverse reactions, Reliable adherence to treatment.
The family consents to the GP taking on prescribing responsibilities under a shared model.
The GP agrees and confirms their capacity to participate in shared care.

Shared Care Pathway

1. We Provide

Diagnostic evaluation tailored to presenting concerns (e.g., ADHD, Autism, or general psychiatric needs). Discussion of treatment recommendations, including both non-medication and pharmacological options.

2. Medication

Medication is initiated following shared decision-making with the family. Baseline physical health metrics recorded (e.g., weight, BP, pulse). Psychoeducation around medication, side effects, and expected outcomes.

3. Stabilisation

The patient attends at least three consecutive review appointments. Monitoring of: symptom response, side effects, dosage adjustments, and engagement and adherence.

4.Review

Clinical team confirms stability and appropriateness for shared care. Shared Care documentation is prepared, including: clinical summary, medication regime, monitoring plan, and risk considerations.outcomes.

5.Handover

Formal Shared Care Agreement sent to the GP. Upon acceptance, the GP assumes prescribing responsibilities. Minds Matter remains available for: periodic specialist reviews, emergency clinical support, and re-referral if symptoms re-emerge or escalate.

Additional Notes

  • Shared Care is not automatic and must be clinically appropriate, safe, and mutually agreed upon by all parties.

  • If a GP declines shared care, Minds Matter can continue prescribing privately.

  • Minds Matter remains clinically responsible for the diagnosis and treatment plan, even after handover.

  • Families are expected to remain engaged and continue reviews as advised.
